Application Process

Our Application Form is very short & easy to complete. We require just a few details and some evidence before we can process your order.
We require the following ID:
ID 1 – Formal Photo ID
- Drivers Licence
- Passport
- Bus Pass
- If you don’t have any of these forms of ID, please contact us and we can discuss another way of identity verification
ID 2 – Evidence of Eligibility
- Carer’s Allowance
- Contract of Employment
- Payslip
- Employer’s note
- Doctor’s note
- Note: Unpaid carers can use evidence from the person they care for (PIP, Disability Allowance etc.)
Evidence must reflect your current position in the social care sector/the role you wish to be printed on The Blue Sky Social Care Card)
Evidence must be no more than 3 months old – exceptions apply
If you are self-employed, we understand that self-employed individuals do not hold the same evidence as others in the social care sector.
Would it be possible to evidence anything like your
- Public liability insurance
- Unique Tax Reference
- An invoice without client details
ID 3 – Photo to use on your card
You will need to upload a clear photo of yourself that we can use on your card.
Employer’s Logo
You can upload your employer’s logo (optional)
